The Geriatrics and Extended Care (GEC) Manager and Palliative Care Champion, referred to hereafter as GEC Manager. The incumbent is a professional nurse accountable for primary assistance to the VISN 21 Chief Medical Officer for delivery of clinical care and the oversight of geriatrics personnel and resources within the VISN. The incumbent is also responsible for coordination of performance improvement, utilization review, risk management, and external review activities at VISN 21 Geriatrics and Extended Care facilities, including community living facilities (also known as NHCUs), community nursing homes and state veterans' homes. Further, the incumbent serves as the VISN Clinical Champion for Palliative Care.

The GEC Manager: Provides executive leadership, oversight and facilitates overall performance improvement activities within the VISN 21 GEC programs. Works towards achievement of high quality, safety, and cost effective evidence based outcomes on a continual journey of a high reliability organization. Collaborates with both administrative and clinical services to monitor quality and safety throughout the VISN 21 GEC programs. Functions as facilitator in implementing action plans and process improvement strategies at the VISN and medical facilities for follow up to include regular progress reports. Reviews Performance Measures, EPRP, VASQIP data, IPEC, and other quality data for trends and early intervention. Applies clinical performance improvement methods, collects outcome data related to policy, procedure, and regulatory compliance and in collaboration with management, initiates performance improvement monitoring projects. Utilizes data and comparative analytical processes in recommending design and implementation of innovative programs to improve service excellence, customer satisfaction, and compliance to patient care and regulatory standards.

  • Demonstrates the ability to effectively utilize communications, resolve problems, make decisions, and manage conflict to enhance all relationships, i.e., patients, staff, families and volunteers.
  • Establishes effective relationships within the Geriatric and Extended Health Care staff throughout VISN 21.
  • Keeps the Chief Medical Officer informed of all matters which may have potential impact on the VISN 21 GEC programs.
  • Adheres to personal and professional obligations and responsibilities, with emphasis on practicing and maintaining ethical standards.
  • Keeps V21 Nurse Executive Committee members informed about special projects and issues affecting nursing staff through formal and informal mechanisms.
  • Conducts meetings using time effectively by defining problems, creating and maintaining common interests, stimulating discussions, directing efforts toward appropriate conclusions, and demonstrating closure on projects.Agendas will be available for meetings and meeting minutes will be completed in accordance with VISN policies.
  • Maintains self-control and responds consistently to the requirements of the work situation.
  • Collaborates with health professionals in facilitating patients' access to the appropriate level of care and care settings based on assessed need for services.
  • Establishes effective communication with external review organizations.
  • Acts as a facilitator for problem and program resolution and program implementation.
  • Acts as a consultant in clinical practice in interpreting and implementing policies and procedures, standards of practices, and external standards of practice.
  • Systematically evaluates current clinical practice based on internal and external regulatory requirements.
  • Recognizes the need for consultation and utilizes clinical expertise of Nursing and Medical Specialists and other disciplines to meet GEC patient and staff needs.
  • Facilitates the delivery of patient care in collaboration with all disciplines within VISN 21.
  • Promotes quality in clinical practice by collaborating in the development and implementation of policies, procedures, programs, and plans for care delivery.
  • Demonstrates the knowledge and skills for provision of appropriate care for geriatric patients and in designing performance improvement activities.
  • Utilizes clinical expertise in acute and long term care in overseeing case reviews, as well as the screening and evaluation of processes of admissions and discharges.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      • Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience.
      • Nurse V - Master's degree in nursing or related field with BSN (or a Bachelor's degree in a related field) and approximately 5-6 years of nursing practice/experience; OR a Doctoral degree (preferred) in nursing or related field and appropriate basic or advanced certification desired, and approximately 4-5 years of nursing practice/experience.
      Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

      Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. In the performance of duties, the incumbent may be required to drive and/or ride in GSA-vehicles.
