Duties

The Geriatrics and Extended Care (GEC) coordinator position is assigned to the Department of Veteran Affairs, Rocky Mountain Network (VISN 19) and reports to the VISN Chief Medical Officer. The Geriatrics and Extended Care Specialist/Coordinator is a Registered Nurse who serves as the primary clinical resource and subject matter expert for the VISN regarding scope and direction of all related geriatrics services and ensures alignment with VISN goals and VACO program offices through coordination and collaboration among facility geriatric program leads, facility clinical leadership, and nursing. The GEC Specialist/Coordinator coordinates, organizes, and administers knowledgeable oversight in geriatrics, contract nursing homes, state Veterans homes, home based primary care, palliative care, non-institutional care and community living centers throughout the 8 medical centers and all facilities in VISN 19. Provides oversight of accreditation programs, makes recommendations for effectiveness, and ensures that programs are designed to provide Veteran centric care with a focus on whole health and seamless care coordination. Collaborates with VISN Primary Care and Mental Health Leads, Facility HBPC, and Geriatrics Leads, State Veterans Homes liaisons, contract Nursing Home coordinators, National Program offices, Chiefs of Staff, Nurse Executives, and VISN Quality Management staff.

Work Schedule: Monday through Friday, 8am to 4:30pm

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • License/Registration: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      Preferred Experience for the Geriatrics and Extended Care Specialist/Coordinator:
      • Experience in geriatrics clinical and program administration.
      • Knowledge of inpatient and outpatient Geriatrics programs.
      • Experience as a Geriatrics Service Lead at a facility or VISN level.
      • Demonstrated strong leadership skills and ability to lead a diverse interdisciplinary group of healthcare professionals, utilizing evidence based and population based approaches.
      • Strategic insight into VHA geriatrics issues and solutions and a strong working knowledge of the VA Geriatrics programs oversight requirements.
      • Demonstrated strong leadership and guidance

      Grade Determination - Nurse IV: The following criteria must be met in determining the grade assignment of candidates:

      Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience.

      Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.
