Why Mayo Clinic
Home Health and Hospice serves 400-500 patients in our community. Staff work in an interdisciplinary team that includes nurses, social workers, chaplains, home health aides, therapists (OT, PT), therapy aides, massage therapists, music therapists, bereavement counselors, physicians, nurse practitioners and volunteers to meet the needs of our patients. The department cares for those with healthcare needs in their home, which includes from birth to death and all types of medical conditions. The patient population is one of the most varied populations found in healthcare!

The Home Health Aide will provide personal care to home health or hospice patients in their home or where the patient calls home. Companionship services are provided as directed by the plan of care for support to the patient and family.

This position will travel and provide services throughout the NWWI region.
Position description
The Home Health Aide/Hospice Aide is a trained person who provides personal care and assistance in activities of daily living for people in their place of residence. These services are included in a written plan of care prepared by a health care professional and signed by a physician. The Home Health Aide/Hospice Aide works under the direct supervision of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist under the delegation of their License. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.
High School Graduate or GED equivalency preferred. Meets Federal and State guidelines required to work as a Home Health Aide in a certified licensed Home Health agency.
Additional qualifications
Certified Nursing Assistant experience within the past 24 months preferred.18 years or older preferred.

Skills Needed: Ability to read and communicate effectively in English. Basic computer skills and experience with other technological devices. (i.e. cell phone, black berries, etc.) Ability to work irregular hours (days, evenings, holidays, weekends). Self motivated. Able and willing to drive in all weather conditions and cover the assigned service area. Able and willing to work with all populations served.
License or certification
Certified Nursing Assistant on State of Wisconsin Registry. Current BLS (Basic Life Support) certification or the ability to be certified prior to start. Valid driver’s license, reliable transportation, the ability to transport patients and acceptable driving record required.

Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $12.35 per hour.

Schedule details
This position works 3 days per week, 8:00am - 4:30pm.
Weekend schedule
Every other weekend and rotating holidays (2 holidays per year). On call required one Saturday or Sunday per month from 8:00am - 8:00pm.
