Responsibilities: This position is part of the Rochester Mayo Clinic Hospice program but provides care primarily for the NE Rochester and Wabasha area with travel both in Rochester and Wabasha (Mileage is reimbursed).

The Hospice Patient Care Coordinator (Registered Nurse) is accountable for the delivery of nursing services based on initial and ongoing assessments of patient needs and in accordance with the interdisciplinary team plan of care. The delivery of nursing services includes direct care, patient/family/caregiver support, instruction and education. In addition, the RN provides telephone triage and on-call responsibility 24/7. The ANA Standards of Clinical Nursing Practice and the Standards of Practice for Hospice Programs provide a basis for the practice of the RN in hospice. The RN supports professional nursing practice across practice settings by providing holistic, family-centered care in efforts to improve quality of life. The RN Care Coordinator provides care wherever the patient is located including the patient’s home, a facility or a Mayo Clinic Hospital.

Qualifications: Graduate of an Accreditation Commission for Education in Nursing (ACEN) accredited associate’s degree or diploma program.

Exceptions to this may be made in the MCHS sites prior to a law change requiring accreditation expected in 2018.

If graduated from a nursing program that was not accredited by ACEN, at least one year of RN experience (within the last 5 years) in an applicable care setting is required.

If graduation did not occur within the last two years, one year of RN experience in an applicable care setting is also required.

Rochester campus: All entry-level associate degree registered nurses with a RN start date after September 1, 2010, must provide documented evidence of program completion of their baccalaureate degree in nursing from an accredited nursing program within 6 years from the last day of the month of their RN start date. All entry-level associate degree registered nurses with a RN start date of September 1, 2014 and after must provide documented evidence of program completion of the baccalaureate degree in nursing from an accredited nursing program within 5 years from the last day of the month of the RN start date.

A driver’s license and mode of transportation is required as the position requires daily travel to the patient’s place of residence.


Additional Qualifications: BSN Preferred. Minimum of two years previous RN experience in acute care, home care and/or long-term care settings preferred. Possesses strong verbal and written communication skills Ability to adapt to unpredictable situations within the home setting. Computer and keyboarding skills preferred.

License or Certification: Must obtain prior to start date:

Current RN license by applicable state requirements.
Arizona and Florida: Maintains Basic Life Support (BLS) competency.

Midwest: Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:
  • American Heart Association (Title: BLS for Healthcare Providers or BLS provider)
  • Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
  • Military Training Network

Additional specialty certification/training as required by the work area.

Exemption Status: Non-exempt
Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $29.95 per hour.
Hours/Pay Period: 48 hours per 2 week pay period
Schedule Details: 8 hour shifts Monday through Friday 8am-5pm
On-call every other weekend
Occasional on-call weekday night shifts

Weekend Schedule: On-call every other weekend 5pm Friday to 8am Monday.
