Summary

Assists RNs in implementing plans of care, prioritization of client needs, relevance of nursing diagnosis, individualization of interventions, expected veteran-resident outcomes, and quality of documentation. Assists NM in promoting professional development and motivation for employees to work at optimal efficiency. Ensures relevant training is completed Assists RN in through collaboration with peers and other healthcare professionals in coordinating care. Assists RN in delivering nursing care. Demonstrates sound clinical judgment in making decisions. Administers medications as ordered or collaborates with RN and/or NP/MD if he/she suspects a medication should not be administered. Assists NM and ANM in compiling data in a timely manner including but not limited to: performance improvement / quality improvement reports; schedules; certified time; workload requests; and patient care data reviews.
Assists NM/ANM in completing special assignments and is a contributing member of Nursing Service. Participates in committees and working groups that work on improving the quality of care and/or the working conditions with-in the facility. Documents patient care according to prescribed procedures, and maintains an objective relationship with the peers and residents. Supports and participates in open communication with coworkers and residents. Able to recognize and respond to urgent needs appropriately utilizing critical thinking skills. Incumbent utilizes good communication skills to identify and resolve problems/issues on the unit. Updates the ANM/RN when appropriate with accurate and necessary information regarding issues that have occurred. Participates in career development through periodic need assessments, and scheduling of training / in-services, continuing education programs, and participation in committees and councils/meetings. Incumbent ensures completion of annual mandatory training requirements and competency statements. Assists in the coordination and orientation of newly assigned staff and Participates in care-planning process for assigned resident. Provides wound care as directed by RN, NP, MD.

Work Schedule: Monday - Friday 7:30am-4:00pm (extended hours and a change in duty location may be required due to patient needs). On occasion may require rotating shifts, some weekends and Holidays.

Duties

As an integral part of the Patient Aligned Care Team (PACT), you will demonstrate sound techniques in complex nursing care procedures and basic care of the patient in the Primary Care Clinic setting. You will provide nursing care in a self-directed manner with work accomplished requiring minimal review by the RN in charge and is administratively responsible to the Nurse Manager or designee for the quality and quantity of work performed on a daily basis, and is responsible and accountable for his/her individual practice. You will complete a focused patient intake and document appropriately. You must possess excellent communications, interpersonal, customer service skills. In addition to providing clinical support, you will be responsible for actively participating in staff meetings and/or workgroups, clean and prepare exam rooms, assist the physicians with procedures and close open encounters, recognize and appropriately respond to breakage/malfunction or loss of equipment.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Education. Graduate of a school of practical or vocational nursing approved by the appropriate State [ ] agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant.
      • Licensure. Full, active, current and unrestricted licensure as a [ ] licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
      • English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

      Grade Determinations:
      In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
      GS-3. None beyond the basic requirements. (LVNs at this grade level are expected to perform all duties within the scope of their license; however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.
      GS-4. Six months of qualifying experience as an LPN or LVN; or
      Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months duration.
      LVNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedures, and techniques.
      GS-5.Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.
      GS-6.Completion of at least 1 year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.

      The full performance level of this vacancy is GS 6. The actual grade at which an applicant may be selected for this vacancy is in the range of GS 3 to GS 6.
