Licensed Vocational Nurse - Ambulatory Care
Los Angeles, CA
Duties:

 

The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking experience Ambulatory Care Licensed Nurses (LVNs) to work in our Primary & Ambulatory Care Clinics located in West Los Angeles, Downtown Los Angeles, and the Sepulveda Campus.  He/she functions as a member of the Patient Aligned Care Teams (PACT) and will be responsible to assist with the day-to-day operations of the clinic.  The Ambulatory Care LVN will ensure that Veteran-centered healthcare is provided and delivered in a quality, timely, accessible and respectful manner, consistent with the missions, goals and objectives of the Veterans Health Administration.

 

 

Include but are not limited to the following:

1. Delivers a broad range of vocational nursing care to a group of patients in a variety of settings.

2. Monitors patients for illness or incapacity.

3. Evaluates and observes patients, identifying obvious and subtle deviations.

4. Recognizes emergent situations and initiates appropriate interventions.

5. Uses practical knowledge of human motivation behavior and reactions to situations.

6. Acts as a patient advocate in facilitating patient care.

7. Exercises self-direction in performing the most complex LVN assignments involving both standard and non-standard specialized activities

8. Participates in identifying, developing, and conducting self-reliance health related teaching programs for patients and families.

9. Contributes to the interdisciplinary team in developing, implementing and evaluating patient treatment plans.

10. Maintains and orders adequate supplies, drugs, and equipment necessary for clinic functions.

11. Participates in clinic Quality Assurance activities.

12. Assists in maintaining accurate logs and statistics as required.

13. Reviews patient records for preventive care, tracts preventive care and conducts patient education associated with chronic disease.

14. Performs other duties as assigned depending on nursing care demands and/or needs.

15. Performs blood draws, EKGs, medication administration and implementation of orders for patient care in select areas.

16. Prepares rooms and patients for examinations and takes responsibility for coordinating smooth patient flow in the clinic area.

 

Duty Location: Will be at one of the following depending on facility needs ~

1. West Los Angeles Medical Center ~11301 Wilshire Blvd Los Angeles CA 90073

2. VA Sepulveda Ambulatory Care Center ~ 16111 Plummer St North Hills, CA 91343

 3. VA Los Angeles Ambulatory Care Center ~351 East Temple Street Los Angeles, CA 90012

 

Duty Schedule: Monday - Friday 8 hour shifts up to 5:30 pm (No weekends or holidays)
