Position Summary: Together Holmen and Arcadia clinics have 8 Family Medicine doctors, 1 Family Nurse Practitioners, 1 Certified Nurse Midwife/Family Nurse Practitioner, a Physician Assistant, and a Pediatrician who provide both preventative health maintenance and chronic disease management services to patients of all ages. Our teams practice the Mayo Model of Community Care and are dedicated to achieving the highest quality of care by ensuring all patients receive the appropriate preventative and diagnostic screenings, follow-up, and patient education.

With management who carries out servant leadership, you will be joining our teams of providers, Registered Nurses, Licensed Practical Nurses, Certified Medical Assistants, and receptionists, along with our laboratory, radiology, and outreach service departments. You will find yourself working in an environment where we focus on collaboration and team commitment.

Qualified candidates must be flexible and have the ability to prioritize; possess a strong work ethic; work closely as a team; approach any situation with a positive attitude, and deliver compassionate care with the primary focus on the patient and their family. These candidates must be willing to flex between both campus locations; have availability to work at least two shifts a week, and have the capacity to commit to planned and unplanned staffing needs.

Responsibilities: The LPN provides basic nursing care to assigned patients under the direct supervision of the registered nurse. The LPN collects data through observation and communicates information to assist the RN in patient assessment and care planning. The LPN is expected to deliver care in accordance with the patient care plan and standards of care. The LPN accepts delegation from the RN in meeting learning/information needs of the patient/family.

Qualifications: Currently licensed or eligible for licensure as a Licensed Practical Nurse in the state of Wisconsin, Minnesota or Iowa, depending on where they are employed or graduation from an accredited medical assistant program and must be certified. Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:

• American Heart Association (Title: BLS for Healthcare Providers)
• Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
• Military Training Network

Additional Qualifications: One year LPN experience within the last five years preferred. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, nights, holidays, weekends, and on-call. Ability to adapt to unpredictable situations within the work setting. Graduate of an accredited school of practical nursing required.

License or Certification: Current LPN license by applicable state requirements or current CMA certification. Maintains current Basic Life Support (BLS) for Health Care Providers.
