Responsibilities: The LPN in the Anticoagulation Clinic will be accountable for the delivery of direct and indirect patient care to patients who need medication dose management, education, and follow-up. The LPN will work closely in a centralized practice providing this service to patients from several Primary Care locations and multiple physician practices to provide high quality systematic care and the promotion of patient/family active involvement in care. The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects. (007734)
Qualifications: Student or Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g. some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
Additional Qualifications: Previous experience in Primary Care preferred, geriatric experience preferred, experience with outpatient anticoagulation management is preferred. Applicants to this position must have exceptional critical thinking, math, and computer skills. LPN’s in this position will need to be flexible, have the ability to multi-task, and work independently. LPN’s in this position may provide occasional cross-coverage to Kasson Anticoagulation Clinic.

License or Certification: Must obtain prior to start date:
Current LPN license by applicable state requirements.
Arizona and Florida: Maintains current Basic Life Support (BLS) for Health Care Providers.
Midwest: Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $19.11 per hour.
Hours/Pay Period: 48 hours per 2 week pay period
Schedule Details: Monday – Friday; Extended Days; 4, 6 & 8-hour shifts, some shifts until 7:00pm – NOT NEGOTIABLE
