 The James J. Peters VA Medical Center, Bronx VAMC affiliated with the Mt. Sinai School of Medicine (NYC), numerous Nursing Academic Affiliations and the Hospital for Special Surgery, is located in the NW Bronx on a self-contained campus easily accessible by car (free parking for eligible employees) and public transportation.

 

We are currently recruiting for 1- FULL TIME Nurse Practitioner VN-610, for the Home base Primary Care unit of the Extended Care Program. Tour of Duty:

Monday through Friday 8:30am-5:00pm.

 

The vacancy is located in Home Base Primary Care Unit of the Extended Care Program.

 

Preferred Qualifications:

MSN with experience in home care and geriatric population preferred.

 

Duties:

 

The HBPC NP functions as primary care provider for the assigned patients. The NP utilizes advanced practice nursing skills and knowledge in a relatively independent manner in areas of patient care, education, research and consultation. The NP collaborates with interdisciplinary healthcare team members on a local and national level to identify, implement, test, and export novel approaches to optimizing quality of life of older veterans. The HBPC NP conducts home visits to assess needs of patients and their caregivers. Performs comprehensive medical assessments/ evaluations in the patients’ home setting with consultation with Medical Director, as needed. Performs medication reconciliation in home settings. Manages and treats medical conditions in the home setting, involving Medical Director as needed. Provides health education to patients and their caregivers. Refers patients to appropriate community or VA programs. Participates in weekly HBPC team meetings and provide innovative management ideas for patient case reviews.
