Summary

The Registered Nurse for the Primary Care Mental Health (PC-MH) and Mental Health Outpatient Clinic at Raleigh. The Registered Nurse will function as the Care Manager for the PC-MH Team. The PC-MH Nurse works closely with the Psychiatrist, Nurse Practitioner, and other service providers to help foster independent living through appropriate management of the client's mental illness by providing assessments, planning, interventions and evaluation of treatment. Mental health treatment provided is documented within prescribed guidelines in CPRS.

Work Schedule: Primarily 8 hours Day tours Monday-Friday. Scheduled rotations to include Weekends and Evening hours to meet Veterans Access Needs as deemed necessary (Advanced notification will be provided).

Duties

MAJOR DUTIES AND RESPONSIBILITIES:
Assist Veterans in the following ways:
Provide Veterans and their family members with education regarding their illness, medications and treatment plan and serves as the Primary Contact for the Veteran.
Provide outreach to clients who are having problems with treatment adherence and who may be relapsing.
Coordinate with other agencies and make referrals to provide patients with necessary community services.
Refer to, consult with and coordinate care with other VA health care providers.
Assess patients and provide additional support between visits.
Facilitate psycho-educational and support groups.
Act as liaison to Inpatient Psychiatric Unit to facilitate continuity of care.
Provide coverage as needed for other staff in the Outpatient Mental Health Clinic.


Assists in scheduling, re-scheduling appointments, and appointment cancellations. Program Support
Performs assessment/triage of Veterans for appointments, phone-calls, and/or walk-ins.
Performs suicide risk assessments and/or mental status exams.
Provides crisis assessment/intervention.
Provides follow-up care, medication evaluation assessment, and administration of medications as needed.
Enters order for medication refills, laboratory, and consults per Policy.
Provides case management for high risk Veterans.
Engages in end-of-life care for Veterans with serious mental illnesses.
Reviews and completes Clinical Reminders per Policy.
Monitors Performance Measures, wait times, incomplete encounters, and outcome measures.
Follow-up phone calls, secure messaging.
Takes vital signs and completes medication reconciliation.
Documents care and treatment planning in CPRS.

    • Basic Requirements:
      • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
      • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
      • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
      Preferred Experience:
      • Knowledge of the Case Management model to interdisciplinary care.
      • Minimum of 1-year acute psychiatric care setting to include SOAP (subjective, objective, assessment and plan) documentation.
      • Minimum of 2 years successful nursing practice.
      Preferred Certification (s):
      • ANCC Psychiatric- Mental Health Nursing
      • ANCC Nursing Case Management

      Grade Determinations: Salary will be determined by a Professional Standards Board.

      Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

      Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position involves considerable standing, walking, stooping, bending, reaching, lifting, sitting, and turning. The work may involve going up and down flights of stairs. The position requires visual and auditory acuity at a level sufficient to provide safe and effective nursing care. The incumbent should have the ability to physically control or protect him/herself from physically abusive patients.
