Registered Nurse - Home Based Primary Care (HBPC) - Bellevue CBOC

Duties:

Position Summary:

The Staff Nurse in Home Based Primary Care (HBPC) provides out-patient clinic services for patients.  They assume accountability and responsibility for his/her nursing practice and decisions affecting patient care. This RN will work with INTOC (In Home Transitions of Care).

 

Duties may include but are not limited to:

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Administers medications including IVP and IVPB and procedures per established policies, protocols and guidelines.
  • Functions as a member of an interdisciplinary team providing patient care in the home.
  • Responsibilities include providing clinical oversight for License Practical Nurses on the team, in home physical assessments,
  • Point of Care testing and phlebotomy in the home, providing wound care and catheter care.
    Will assess the status of Home Care patients which will include planning, making appropriate interventions and ongoing evaluation of the patient.
  • Functions as a Case Manager and coordinates the patient care with interdisciplinary team members and Nursing staff of outside agencies who may be providing additional nursing, hospice and IV therapy services to the patient.
  • Considers all characteristics of the individual, including sex and sex role identification, age and life stage, state of health, race and culture, socio-economic status, values and previous experience with similar events, in order to assist each patient to identity and meet basic health needs and improve his/her health status to the individual's maximum potential.
  • Functioning as a preceptor assumes accountability and responsibility for the clinical development of assigned orientees.

 

Tour of duty: Primarily Monday - Friday, 8:00am-4:30pm
