Responsibilities: Reviews all documentation for agency admissions, resumption of care, recertification’s/follow up, transfers and discharges to ensure accuracy, congruency and accurate comprehensive assessment for quality reporting and billing. Reviews all Home Health & Hospice Quality dashboards, reports and quality work queues. Responsible for review of OASIS documentation to ensure accuracy for Home Health regulatory compliance, Billing requirements, and quality of care. Completes Quality review of all Hospice documentation to ensure accuracy of Hospice regulatory compliance, Billing requirements and quality of care. Submits Home Health & Hospice regulatory data (HHCAHPS, HCAHPS, and HIS). Works closely with Supervisor, Director, Professional Staff, Educators, Billing, and Coding. Provides one to one coaching to staff on documentation requirements, deficiencies.

Qualifications: Active WI RN Licensure or eligibility to practice professional nursing in the state of Wisconsin and Minnesota required. Bachelor of Science in nursing preferred. Minimum three years recent nursing experience with one year home care experience required. OASIS certified or ability to obtain certification within six (6) months from the date of hire.

Able to work in a fast paced changing environment. Ability to handle crisis situations effectively and diplomatically. Demonstrates exceptional customer service and organizational skills. Experience in dealing with a diverse range of clients and customers. Demonstrates excellence in the nursing process including assessing, planning, implementing, and evaluation nursing interventions. Basic computer skills
