Position description
The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. This role frequently involves sitting for extended periods of time. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN.
Qualifications
Graduate of a baccalaureate Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience.
Additional qualifications
BSN Required. Must be currently working in direct patient care. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills (e.g. PowerPoint, Excel, Word) preferred, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days and evenings. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making.
License or certification
Must obtain prior to start date:

Current RN license by applicable state requirements.

Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:

•American Heart Association (Title: BLS for Healthcare Providers or BLS Provider)
•Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
•Military Training Network

Additional specialty certification/training as required by the work area.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $33.37 per hour.

Monday – Friday; Days (7am-4pm; 8am-5pm; 8:30am-5:30pm), 8-hour shifts; on-call shifts – NOT NEGOTIABLE

On call 1 of 8 weekends and 1 holiday per year – NOT NEGOTIABLE
