Position Summary: The Transplant Coordinator Nurse is accountable for the delivery of direct patient care and supervises nursing care provided by assistive personnel. Collaborates with nurse education specialists and nurse managers to provide information to the nursing staff. Assesses and plans in collaboration with the patient, significant other and interdisciplinary healthcare team to meet educational and discharge needs. Assesses data reflective of the patient's status and appropriately interprets information relative to the patient's age-specific needs. Provides patient and family education on a consistent basis in the inpatient and outpatient setting. Handles 24/7 on-call responsibilities on a rotating basis. Weekly duties include but are not limited to inpatient, clinic and triage responsibilities. Provides leadership through activities such as community involvement, committee work and participation in research. The American Nurses Association (ANA) Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses and the United National Organ Standard's guidelines provide the basis for practice.

Coordinators in Bone Marrow Transplant must obtain Adult or Pediatric Oncology Certification (OCN or CPHON), BMTCN or Certified Hematopoietic Transplant Coordinator (CHTC) credential within three years of hire.

Responsibilities: The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected.

Qualifications: Graduate of a baccalaureate National League for Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience.

Additional Qualifications: Population specific experience preferred. Knowledge of population characteristics and desired outcomes. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills (e.g. PowerPoint, Excel, Word) required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making.

License or Certification: Current Arizona RN Licensure required. Maintains Basic Life Support(BLS) competency. Basic Life Support certification is required upon hire and must be valid for 90 days or greater from start date. Additional specialty certification/training specific to care coordination as required by the work area.

Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $2,860.00, based on a full-time position.
Hours/Pay Period: 80
Schedule Details: Monday through Friday.
