We are looking for a Registered Nurse to become part of the Home Based Primary Care team in the Binghamton area. The position is located in the Community Based Outpatient Clinic, 203 Court Street, Binghamton, NY 13901. The Registered Nurse in this position provides health care services and care management to high risk patients in their homes. The Home Care Nurse is accountable for meeting and maintaining the qualification standards and functional statements of assigned grade. Our facility is seeking individuals who possess good organizational skills which enable completion of work, demonstrated ability to assume leadership in a patient centered program and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. He/she assesses, plans, implements and evaluates patient care needs assuring quality of care. Provides professional nursing care that includes, but is not limited to:

  • Case Management.
  • Patient Assessments.
  • Participation in interdisciplinary team process.
  • Home care visits.
  • Performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care).
  • Completes and submits clinical notes.
  • Observes patient for changes in conditions and reports appropriately.
  • Provides education instructions to patients and their designated significant other.
  • This position requires that the employee operate a government motor vehicle.
  • Performs other professional nursing duties as assigned and within their scope of practice.

Work Schedule: 1st Shift 8:00 am to 4:30 pm

    • Basic Requirements:
      • Citizen of the United States. Noncitizens may be appointed when it is not possible to recruit qualified citizens.
      Graduation from School of Nursing. Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed by the applicant.
      • The Accreditation Commission for Education in Nursing (ACEN) Or
      • The Commission on Collegiate Nursing Education (CCNE), an accrediting arm of the American Association of Colleges of Nursing (AACN).The CCNE accredits bachelors and masters degree programs.
      OR
      The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded.
      OR
      In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

      Registration.
      Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.

      Impaired Registration. ny registration(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status.A registered nurse who has or ever had any such impairment to their registration as listed above may be appointed only in accordance with the provisions of chapter 3, section B, paragraph 15 of this part.

      English Language Proficiency.RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

      Preferred Experience: In addition to meeting one of the educational requirements above, the candidate must have 2-3 years of home care experience, or an MSN and approximately 1-2 years of home care experience; Masters in a field related to nursing and a BSN and approximately 1-2 years of experience.

      Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
      Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
      Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
      Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
      Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
      Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.
